⚠️ Parental Consent & Liability Waiver

PARENTAL CONSENT AND RELEASE OF LIABILITY AGREEMENT

I, the parent or legal guardian of the minor named above, hereby grant permission for my child to participate in activities at Usurp Gaming Lounge located in East Northport, NY.

Acknowledgment of Risks: I understand and acknowledge that participation in gaming activities involves certain inherent risks, including but not limited to: use of electronic equipment, interaction with other patrons, consumption of food and beverages, and general movement within the facility.

Release of Liability: In consideration for allowing my child to participate, I hereby release, waive, discharge, and covenant not to sue Usurp Gaming Lounge, its owners, employees, agents, and representatives from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child while participating in activities at the facility, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Medical Treatment: I authorize Usurp Gaming Lounge staff to seek emergency medical treatment for my child if necessary. I understand I will be responsible for any medical expenses incurred.

Conduct and Rules: I understand my child is expected to follow all facility rules and staff instructions. Usurp Gaming Lounge reserves the right to remove any participant who violates rules or engages in unsafe or disruptive behavior, without refund.

Supervision: I understand that while staff will be present, Usurp Gaming Lounge is not a supervised childcare facility. My child is responsible for their own behavior and belongings.

Property Damage: I agree to be financially responsible for any damage to equipment, property, or furnishings caused by my child, whether intentional or accidental.

Content Rating: I understand that games with various ESRB ratings may be available. I acknowledge my child may be exposed to age-appropriate gaming content and interactions with other patrons.

Photography/Video: I consent to my child being photographed or recorded during their time at the facility for promotional purposes, unless I notify management in writing.

Legal Authority: I certify that I am the parent or legal guardian of the minor named above and have full legal authority to execute this agreement on their behalf.

Severability: If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY.
